Coupling

I have two different physics in one model (one is convection and diffusion, the other is just diffusion). I am trying to model dialysis and I have no flux equations, so I need to set the concentrations at the adjacent boundaries equal to each other. I can not for the life of me figure out how to do this. Thank you for your help.
